Request fit to travel certificate

Thank you for your application for a fit to travel certificate. The assessment will be done by answering the questions below. Our physician will evaluate your request after which the certificate will be issued or you will be invited for a medical consultation.

The Thai authorities do not accept certificates that are older than 72 hours. We kindly ask you to:

Send us the reply with answers 4 days before the departure date at the earliest, later is allowed. Example: you travel on Friday? You can send the answers on Tuesday or later.

Not to call us with a request to send your certificate prior then 72 hours before departure, we are not allowed to do so. Example: you travel on Friday? You will receive your certificate on Wednesday or Thursday.

In the past 10 days, have you been diagnosed with COVID-19 or had a test confirming you have the virus?

In the past 14 days, have you had "close contact" with someone who was diagnosed with COVID-19?

In the past 24 hours, including today, have you had one or more of these symptoms?

- Fever (100.4 F/38.0 C or greater), chills, repeated shaking/shivering
- Cough
- Sore throat
- Shortness of breath, difficulty breathing
- Feeling unusually weak or fatigued
- Loss of taste or smell
- Muscle or body aches
- Headache
- Runny or congested nose
- Diarrhoea
- Nausea or vomiting

PCR Test Only if you are a resident of the country you are travelling to and you are flying there directly, you do not need a negative PCR test. In all other cases you need a negative PCR test. This test has to be taken 72 prior to departure, at the earliest. If you do not upload one now, you will be contacted to do this later.
